Client forms
New to Quiet Light Counseling? You don't need any forms for a free consultation. Paperwork only comes in once we've decided to work together.
How it works
- After we agree to work together, you'll get a secure email from SimplePractice, the client portal I use.
- Open the link and complete each item. You'll see only the forms and consent documents that apply to you.
- Sign electronically. Everything stays inside the secure portal, so nothing personal travels by regular email.
- Once your forms are complete, we'll schedule your first session.

Log in to the Client Portal to finish your forms, see appointments or update your information.
